You can either pootle it
or use the functions perspective, skew and distort in photoshop.

Cheekiest way is to pop a layer underneath the text/image with a rectangle the same size as the bit of the box you want to map it to.
Group this and the text/image layer together.
Free transform both layers (easy now you've got the rectangle for reference - just pull each corner to the 3D box).
Delete the rectangle layer.
Does that make any sense?
