i have a photo which i'm using for refference and when i'm drawing in new areas (like i've started for the neck there) i use a grid
but i'm not tracing it

ears and noses aren't bad...
but eyes and hair are the real tricky parts... it doesn't take much to do an eye, but to do an eye that looks like the REAL eye takes skill

I usually sketch the eyes and nose a bit, then fill in most of the face shadows and highlights, then go back and do they eyes, nose and mouth in more detail - that way you can do the smooth parts of the face quickly without needing to mask out the eyes or anything.
