Isn't the cup sizemeasured in proportion to the chest size? My friend has a girlfriend with 30Fs, and the breasts themselves aren't any bigger than 32D/DD woman would have

yep, that's truebut generally someone who's 30F is going to have a smaller frame to support them, hence the back issues.
