The source pic had already been enhanced somewhat (that explains the chest) but the original was apparently taken at an eating disorder clinic in California (that explains the sun visor..).

As a side note, apparently this picture and others like it were used as 'inspiration' (or as those fruitcakes like to say - 'thinspiration') on Pro-Anorexia websites.
